NorthShore University HealthSystem is gathering information as part of developing a plan to improve health and quality of life in the community it serves. We are reaching out to community members to gather input so we may identify and respond to unmet health needs in the community. Your knowledge and familiarity with services offered by NorthShore University HealthSystem will provide valuable insight into how we can better serve patients in the future.
The survey consists of questions in two parts. Questions in Part I are directed to your Individual health status. Questions in Part II are directed to the Community’s health status.
Key themes that emerge from the survey will be compiled by Crowe, LLP, an independent advisory firm assisting NorthShore University HealthSystem with our assessment. Your responses are anonymous and will remain confidential.
